THIS MARRIAGE LICENSE MAY BE USED ANYWHERE IN THE STATE OF MICHIGAN ONLY
TO OFFICIANTS (JUDGES, MAGISTRATES & CLERGY PERSONS)
1. PRINT OR TYPE at the bottom of BOTH ORIGINAL CERTIFICATES, preferably in black ink.
2. Include the PLACE and DATE of Marriage. The ceremony must be performed AFTER the date issued ON or BEFORE the expiration date on the marriage license, otherwise it will be CANCELLED.
3. Sign and print your NAME, TITLE (do NOT use Officiant or Clergy; acceptable ordination titles include: Minister, Reverend, Imam, Rabbi, Priest, Deacon, etc.) and COMPLETE ADDRESS.
4. Two witnesses over the age of 18 must sign on the signature lines and type or print their names below on the designated lines.
5. The couple must sign on the signature lines and type or print their names below on the designated lines. Although not required, if an applicant intends to change their last name to the spouse's last name, it is suggested to sign the marriage license accordingly.
6. Give ONE ORIGINAL CERTIFICATE to the applicants (as a keepsake only).
7. **It is your legal responsibility to RETURN the SECOND ORIGINAL CERTIFICATE to the Clerk's office within 10 days to the address below (return envelope is provided).
